iCARE Components, Interprofessional Teams, and Patient Outcomes in a VA Hospital Setting

As an LPN nurse at a VA hospital, I have had the opportunity to work with interprofessional teams for close to 10 years. My work has primarily involved caring for patients with chronic kidney conditions (offering dialysis) and HIV/AIDS patients. I am also currently working to switch my career path to mental health. Being posted in a clinic with a high volume of patients from a relatively wide geographic area, collaboration has been a key part of our operation and ultimately patient success.
