Directions for Part 1

  1. Review the Direct Care Project Overview above. Select topic (Air Quality OR Substance Use) from the table below. This will be the topic of your entire course project.
  2. View the Direct Care Project Part 1 TutorialLinks to an external site..
  3. Download the corresponding Assessment and Diagnosis template under Templates below. For example, if you have selected Air Quality as your topic, download the Air Quality Assessment and Diagnosis Template.
  4. Collect the data outlined in the template. There are two parts:
    • Online data collection – gather data from the weblinks provided on the template
      • Please note, if you live in Washington D.C. it may be listed as a state or county. You may list the Washington D.C. data as state and county, then compare to the U.S. data.
    • Modified windshield survey data collection – gather local community data by physically looking at your community
      • If possible, plan on asking someone to drive during your survey so that you can take notes.
      • Your community should be the area where you live or the area surrounding your work. The community must include a residential area and be a large enough area to answer the survey questions. If you live in a large area, you may choose a suburb.
      • Note: It is helpful to conduct this portion of the assessment at least two different times: during the day or evening, on a weekday, and/or on the weekend.
  5. Identify a nursing problem (nursing diagnosis) based on data and observations
    • You must select a problem caused by your topic
    • Important: the problem you identify will be utilized in Parts 2, 3, and 4 of the project.
  6. Identify the vulnerable population that the problem affects. This will also determine who you will present to in Part 3.
  7. Write the nursing diagnosis (See template and chapter 6 in Nies & McEwen textbook for more information)

Directions for Part 2

Screening, Brief Intervention, and Referral to Treatment (SBIRT): Planning the Presentation Instructions

Directions: Create a PowerPoint presentation teaching about the use of Screening, Brief Intervention, and Referral to Treatment (SBIRT). See the assignment directions and tutorial for suggestions and best practices. Review the week’s readings prior to starting on your presentation, understand SBIRT, and its use related to the problem you identified in Part 1: Assessment and Diagnosis assignment.

  1. Identify at least 3 practicing registered nurses who will volunteer for a session to learn about the SBIRT process that is used to identify, reduce, and prevent problematic use, abuse, and dependence on alcohol and illicit drugs.
    • You can present to all at the same time or individually. Set a date and time(s) for your presentation during Saturday of Week 5 and Saturday of Week 6 to allow ample time to complete the Week 6 Part 4 Evaluation assignment.
  2. Review local data related to the specific problem or addiction identified in the Community Health Assessment and Diagnosis assignment. Maintain privacy and only include data that is available to the public. Include citation(s) on the slide when resources are used. Include all resources used on reference slide.
  3. Download the PowerPoint Presentation Template PowerPoint. (Once your presentation is complete, follow the job aid located in the Canvas Week 4 module Part 2 directions on how to convert the PowerPoint to PDF format with speaker notes displayed).
  1. Create a PowerPoint presentation with speaker notes(8-15 slides not including introduction or references) for fellow healthcare professionals that includes:
  • Community description
    • Basic demographic data (i.e. population, size, education, poverty levels, etc.)
    • Relevant epidemiological data from Community Assessment and Diagnosis assignment.
  • Problem description
    • Comprehensive overview of the substance and problem
      • Overview of the problem and relevant data
      • Give an overview of contributing factors specific to your community (supported by data/ scholarly evidence) (i.e. target marketing, lack of employment opportunities, few treatment providers, lack of transportation, availability, etc.)
    • SBIRT description
      • Elements
        • Description of SBIRT abbreviation and process
          • Include an overview of each part of SBIRT
          • Identify the steps that are used with each part
        • Use
          • How SBIRT is used specific to identified problem
            • e. how to incorporate into an assessment
          • Example
            • How to use SBIRT in nursing practice with vulnerable populations
              • This may include a role play, script for a nurse to use in their assessment, example of an existing tool, etc.
            • Outcomes
              • How SBIRT impacts the problem?
                • e. enhances awareness, motivates change, health improvement, cost reduction, etc.
  • Community Resource
    • Describe one community resource that may assist with the substance use concern.
      • (For example, a local substance abuse clinic).
    • Include how to reach this resource
      • Phone, referral form, etc.
    • Assess 4 A’s: affordability, accessibility, acceptability, availability for each resource (see the Week 4 lesson)
  • Implications for practice
    • Provide a comprehensive overview of the impact of substance use to the audience. What does this data mean to them? How does the data impact them as healthcare professionals?
    • How could the implementation of SBIRT intervention into their practice impact patient health outcomes?
    • Use at least one scholarly reference as support
  • Conclusion
    • Includes brief overview of:
      Community demographic and epidemiological data;
      2. Problem;
      3. SBIRT description;
      4. Resources;
      5. Implications for practice;
      6. Importance to improving population health outcomes
  • Q&A slide
    • Give your audience an opportunity to ask questions they may have.
  • Reference slide
    • Minimum of 3 references. Should include references for SBIRT overview, community data, and at least one scholarly source. Utilize more references as needed.

Directions for Part 3

  1. For Part 3 of the direct care project, you will be presenting the PowerPoint to your identified audience. There is no submission for this assignment. 
  2. Presentation should be scheduled between Saturday of Week 5 through Saturday Week 6.
  3. You may present on more than one date/location to accommodate attendees.View the Direct Care Project Part 3 Tutorial
  1. Think about how you will be presenting the PowerPoint.
    • You may present using a laptop, tablet, or projector.
    • Make sure the presentation is visible to audience.
    • You may print handouts for your audience.
    • Determine what professional attire you will wear to presentation. You are representing Chamberlain University.
  2. Under templates below, download the following:
    • Attendance Form (1 sheet)
    • PRE-Survey (1 per attendee)
    • POST-Survey (1 per attendee)
  3. Modify the templates to fit your topic. See areas in red on the templates.
  4. Print your speaker notes and practice the presentation.
  5. Contact the individuals to confirm the location and time.
  6. Come to presentation prepared with attendance form, surveys, notes, handouts (if needed) and laptop.
  7. Greet your audience professionally.
  8. Have all attendees complete all the information on the Attendance Form.
  9. Have all attendees complete the PRE-Survey prior to the presentation and return to you.
  10. Present your topic and answer any audience questions.
  11. Have all attendees complete the POST-Survey after the presentation and return to you.
  12. Thank the audience for their time.
  13. Save the surveys and attendance form for Part 4: Evaluating the Project.

Directions for Part 4

  3. Complete the Part 4 template with the following:
